Job Summary

Nascentia Health is seeking a highly skilled Registered Nurse Case Manager to join our team. As a key member of our home health care team, you will be responsible for providing direct patient care, coordinating overall home care services, and documenting care for agency clients in accordance with Quality Standards.

Key Responsibilities
  • Ensure quality and safe delivery of home health services, including case managing client caseloads, developing plans of care based on client assessments and OASIS evaluations, and ensuring all documentation meets quality standards.
  • Complete all OASIS evaluations, documentation reflects nursing care and interactions, and documentation is clear, concise, and completed in a timely manner.
  • Coordinate LPN care, provide guidance and education to LPNs, and work in conjunction with direct supervisors and Patient Care Coordinators to efficiently manage patients using a team approach.
  • Supervise paraprofessional staff relevant to patient care, supervise home health aides in performing designated tasks related to patient care, and communicate appropriate information to the home health aide department.
  • Comply with all Nascentia Health policies and procedures, maintain patient/employee confidentiality, and demonstrate knowledge and use of infection control, federal/state/local government rules and regulations.
  • Maintain and update clinical knowledge on an ongoing basis.
Requirements
  • Current New York State registered professional nurse license (degree must be from an accredited school of nursing). BSN a plus.
  • Current and valid driver's license and auto insurance, and a vehicle which can be used for work.
  • Self-directed and organized with the ability to work independently in a fast-paced environment.
  • Previous home care/case management experience a plus.
  • Proficient computer skills (including knowledge of electronic medical records software) with the ability to learn new software.
  • Exceptional oral, written, and verbal communication skills.
Physical Requirements
  • Ability to sit or stand at a desk 90% of the day.
  • Ability to perform tasks involving physical activity, including pushing/pulling on average 50 lbs with a max of 150 lbs, lifting/carrying on average 25 lbs with a max of 40 lbs, patient transfers, extensive bending, standing, kneeling, walking in various weather conditions, and ability to go up and down stairs.
  • Ability to drive in all weather conditions (rain, sleet, snow, wind).
  • Visual/hearing ability sufficient to comprehend written or verbal communication.
  • Ability to use a telephone and communicate over the phone.
  • Frequently uses a laptop computer to enter orders and obtain product information.
  • Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids.
