Job Summary:

As a LPN, you will be responsible for providing high-quality nursing care to our patients in their homes, under the direct supervision of a Registered Nurse (RN).

Key Responsibilities:

  • Provide skilled nursing care to patients in their homes, including curative, rehabilitative, palliative, or preventative nursing care.
  • Work collaboratively with the RN to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, and document findings in a timely and accurate manner.
  • Administer medications and treatments as prescribed by physicians, and monitor patients' responses to care.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current with best practices and industry developments.

Requirements:

  • Current State LPN or LVN License
  • Graduate of an accredited practical nurse or vocational nursing program.
  • At least one (1) year of nursing experience, preferably in community/home health or medical/surgical settings.
  • Valid BLS certification
  • Ability to work independently and as part of a team, with excellent communication and organizational skills.
  • Licensed driver with a reliable vehicle and valid insurance.
