Key Responsibilities- Lead Care Coordination: Fulfills the role of an RCC, completing patient admissions, and serving as a resource to other RCCs.
- Communication and Coordination: Leads communication and coordination of care between all members of the care team.
- Patient Advocacy: Assists patients and care team members throughout the care model by acting as a patient advocate.
- Staffing and Resource Management: Assists the market manager in coordinating staffing and ensuring appropriate resources are available to support patient care.
- Quality Improvement: Identifies and solves negative trends and hurdles involved in healthcare systems and incorporates solutions for system challenges.
- Collaboration and Teamwork: Builds and maintains collaborative professional working relationships with all care team members.
- Recruitment and Training: Assists as needed in recruiting, interviewing, and training new team members.
- Education: Associate of Science in nursing.
- Licensure: Current, active RN state licensure.
- Experience: Three to five years of acute nursing experience, with a general knowledge of clinical standards and outcome measurement.
- Transportation: Reliable transportation and proof of liability insurance.
- Education: BSN preferred.
- Certification: Care Coordination and Transition Management (C.C.C.T.M).
- Regional Knowledge: Prefer knowledge of the geographical area of the assigned market.
- Experience: Three months of experience as an acute care RN providing in-home care.
- Care Coordination Experience: Two to three years of recovery care coordinator experience.
