The Registered Nurse Assessment Coordinator (RNAC) plays a pivotal role in overseeing patient and resident care throughout their journey, from admission to discharge, under the guidance of the Director of Nursing (DON).
KEY RESPONSIBILITIES:
- Assessing and monitoring resident skilled services to ensure quality care.
- Managing the Resident Assessment Instrument (RAI) process effectively.
- Completing a precise Minimum Data Set (MDS) and formulating the interdisciplinary Plan of Care.
- Observing patient/resident conditions to detect changes, identify discharge barriers, and track progress towards care objectives.
- Engaging in daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meetings (ITM).
- Fostering positive relationships with all healthcare team members and collaborating as part of the nursing management team.
QUALIFICATIONS:
- Current, unencumbered RN license in the state of practice.
- Minimum of 2 years of direct nursing experience.
- At least 2 years of experience in long-term care settings.
- Preferably, a minimum of 1 year of experience with RAI.
- Required proficiency in ICD 10 coding.
