We take a holistic approach to patient care, offering services to treat the physical, emotional, and mental needs of our patients and their loved ones. Our team is dedicated to making a difference in the lives of the patients we serve.
Responsibilities
- Perform comprehensive patient assessments to identify individual needs and develop personalized care plans.
- Provide patient visits as scheduled, offering emotional support and guidance to patients and their families.
- Offer short-term crisis intervention and individual or family counseling as needed, providing a safe and supportive environment for patients to express their concerns.
- Utilize our electronic medical record system, Brightree, to complete accurate and timely documentation.
- Serve as a patient and family unit advocate, ensuring their voices are heard and their needs are met.
Requirements
- Masters degree in Social Work preferred, but a Bachelors in Social Work is also accepted.
- Experience in Medical Social Work is preferred, but not required.
- Experience within the Hospice field is preferred, but not necessary.
- Valid driver's license, car insurance, and reliable transportation are required.
- Ability to travel throughout assigned service areas is necessary.
