RESPONSIBILITIES:
As a Registered Nurse Care Assessment Leader (RNAC), your key role will involve assessing the appropriate level of care required for our residents.
You will be responsible for scheduling and supervising all resident MDS Assessments.Participate in the pre-admission evaluations to ascertain the care needs of prospective residents.
Review all MDS assessments and associated documentation to ensure timely completion.
Analyze Quality Indicator reports to identify potential resident issues and concerns.
Contribute to the Quality Assessment and Assurance Program.
Understand and adhere to Resident Care Plans.
Ensure precise documentation throughout your shift.
Complete accurate charting, reporting, and progress notes.
REQUIREMENTS:
Must possess a valid and active RN License in good standing.
Experience in long-term care is preferred.
Previous experience as an RNAC is also preferred.
