Social Services Assistant Job Description

We are seeking a dedicated and compassionate Social Services Assistant to join our team at Sedgewood Center - Falmouth, ME. As a key member of our care team, you will provide administrative support to our social service staff, ensuring a seamless transition for patients and residents throughout their stay.

Key Responsibilities:
  • Collaborate with patients, residents, and their families to provide support and resource information.
  • Respond to issues identified by patients, residents, and their families.
  • Facilitate patient/resident transfer within the nursing center to ensure a smooth transition and adjustment.
  • Participate in the healthcare decision-making process under the direction of a qualified social worker and/or consultant.

Requirements:
