Job Summary:

The Registered Dietitian is a key member of the Elderwood team, responsible for ensuring the nutritional well-being of our residents. As a Registered Dietitian, you will be responsible for conducting nutrition assessments, developing individualized nutrition care plans, and providing education to residents and their families.

Responsibilities:

• Conduct comprehensive nutrition assessments for adult and geriatric populations in a long-term care and sub-acute rehabilitation setting, adhering to Elderwood's policies and standards of care.

• Develop, implement, and monitor individualized nutrition care plans based on current best practices, ensuring residents receive optimal nutrition and care.

• Provide nutrition counseling and education to residents and their families, promoting healthy eating habits and lifestyle choices.

• Maintain accurate and up-to-date resident nutrition information in our computer system, ensuring seamless communication among healthcare teams.

• Conduct routine audits and monitor performance improvement initiatives related to nutrition and food services, identifying areas for improvement and implementing changes as needed.

• Participate in resident care plan meetings, staff meetings, and in-service programs, collaborating with multidisciplinary teams to ensure comprehensive care.

• Stay current with federal and state regulations regarding nutritional services in long-term care settings, ensuring Elderwood's compliance and adherence to industry standards.

• Serve as a nutrition consultant to the Administrator, Diet Technician, Food Service Director, and multidisciplinary medical team, providing expert guidance and support.

• Act as a preceptor for dietetic students and interns, mentoring and guiding them in their professional development.

• Conduct in-service programs for dietary and facility staff, sharing knowledge and best practices to enhance resident care and satisfaction.

• Collaborate with the Food Service Director and Dietary Supervisor to oversee department operations, ensuring seamless execution of nutrition and food services.

Requirements:

• Bachelor of Science Degree or higher in Nutrition/Dietetics from an accredited educational program.

• Successful completion of an internship or equivalent clinical management practice, qualifying for Professional Registration with the Commission on Dietetic Registration.

• Active and ongoing Registration with the Commission on Dietetic Registration.

• Active membership in The Academy of Nutrition and Dietetics preferred.

• Certified Dietitian - Nutritionist (CDN) with The New York State Department of Education or Licensure in the state of employment (or to be obtained within one year from date of hire).

• Long-term care experience preferred.

• Computer skills and knowledge in nutritional software programs preferred.
