About the Role

BAYADA Home Health Care seeks a highly skilled Registered Nurse to join our team as a Nurse Specialist in Hospice Care. This rewarding role involves providing comprehensive care to patients in their homes, promoting comfort, dignity, and quality of life.

Key Responsibilities
  • Develop and implement personalized care plans tailored to each patient's unique needs and goals.
  • Provide hands-on nursing care, including wound management, medication administration, and vital sign monitoring.
  • Maintain accurate records and communicate effectively with patients, families, and healthcare teams.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and transitions.
Requirements
  • A current RN license in Vermont and New Hampshire.
  • A minimum of one year of recent clinical experience, preferably in hospice or palliative care.
  • Prior home health experience is an asset, but not required.
  • Strong communication, interpersonal, and time management skills.
  • Ability to work independently and as part of a dynamic team.
