Occupational Therapist Job Description

Ensign Therapy is seeking a skilled Occupational Therapist to join our team at Fountain Hills Post Acute Care. As an Occupational Therapist, you will work with patients to develop and implement individualized plans of care to help them achieve their goals and improve their functional abilities.

Responsibilities:
  • Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care following all regulatory and clinical practice standards.
  • Utilize standardized assessments and evidence-based practice to support clinical interventions.
  • Provide rehabilitative, skilled, and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
  • Administer various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and the use of supportive and assistive devices.
  • Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluate treatment outcomes for effectiveness and modify services in relation to patient evaluation and referrals.
  • Provide consultation and counseling to patients, as well as families, caregivers, and other service providers related to physical disorders.
  • Effectively communicate with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
  • Provide comprehensive discharge summaries of services provided per regulatory and clinical requirements.
