OUTPATIENT SERVICES COORDINATOR - ENSIGN THERAPY

Ensign Therapy is on the lookout for a dedicated licensed therapist to join our team as an Outpatient Services Coordinator. We are in search of an individual who is deeply committed to their field and takes pride in their contributions. In this role, you will be instrumental in fostering and enhancing relationships with both existing and potential outpatient referral sources through direct engagement with healthcare professionals, office administrators, referral specialists, and community members. Your role will involve educating these stakeholders about the comprehensive services we provide, discussing patient care pathways, and expressing gratitude for their ongoing referrals. As a treating therapist, you will have the opportunity to work with older adults both in our skilled nursing facilities and within the community.

Key Responsibilities:
  • Develop and nurture relationships with current and prospective referral sources through in-person meetings with healthcare providers, office managers, referral coordinators, and community members.
  • Identify and promote our services to local primary care and specialty physicians to meet their patients' outpatient therapy requirements.
  • Assist the Rehabilitation Director in organizing community outreach events, public relations initiatives, or specialized marketing efforts.
  • Oversee the outpatient therapy schedule to ensure the establishment and maintenance of Plans of Care (POC) while coordinating with fellow therapists to reschedule any missed appointments.
  • Deliver therapy services to residents in long-term care, skilled inpatient settings, and outpatient environments.
  • Provide therapy services in patients' homes, group homes, and assisted living facilities.
Qualifications:
  • Licensed Physical Therapist (PT), Physical Therapist Assistant (PTA), Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or Speech-Language Pathologist (SLP).
  • Experience in healthcare marketing is advantageous but not mandatory.
  • Exceptional interpersonal abilities, including the capacity to quickly establish rapport.
  • Outgoing personality with a strong commitment to community involvement.
  • A minimum of one year of experience in skilled nursing facilities and/or home health settings.
