Responsibilities

As a Licensed Practical Nurse (LPN) with CommonSpirit Health at Home, you will have the opportunity to work in a dynamic and fast-paced environment, providing skilled nursing care to patients in the comfort of their own homes. Your responsibilities will include:

  • Providing direct patient care, including administering medications, wound care, and vital sign monitoring
  • Developing and implementing individualized care plans to meet the unique needs of each patient
  • Collaborating with other healthcare professionals to ensure seamless care coordination
  • Conducting thorough assessments and evaluations to identify patient needs and develop effective care plans
Qualifications

To be successful in this role, you will need to have:

  • A graduate degree from an accredited school of practical nursing
  • A current license to practice as an LPN in the state where you will be working
  • Current CPR or BLS certification
  • Compliance with state certification/registration requirements
  • Proof of eligibility to pass medications
  • At least one year of hospital nursing experience

Pay Range

The hourly pay range for this position is $28.22.
