Job Summary

We are seeking a skilled and compassionate Per Diem RN to join our team at Healthfirst. As a Per Diem RN, you will conduct home assessments for prospective members and re-assessments of current members using the Uniform Assessment System (UAS). You will make enrollment and continued eligibility determinations by utilizing clinical expertise and critical thinking skills.

Key Responsibilities
  • Conduct home assessments and re-assessments of current members using the UAS
  • Make enrollment and continued eligibility determinations using clinical expertise and critical thinking skills
  • Complete assessments of service needs and make necessary adjustments to Care Plans
  • Conduct in-person Care Management visits in support of person-centered service planning
  • Liaison between members and Care Management Teams to assess home environment and psychosocial status
  • Provide feedback to home care agencies and Care Management Teams regarding home care issues and contract nurses and aides performance
  • Provide grass roots, community-based training for frail population care including self-care techniques and prevention strategies
  • Ensure members and significant others/responsible parties understand and are in agreement with enrollment in a managed long-term care plan
Requirements
  • New York State RN license
  • Ability to travel to Bronx NY
  • Preferred qualifications include work experience in geriatrics, home care, discharge planning, case management, and/or medical surgical nursing
  • Experience using electronic patient health information (PHI) database usage especially UAS
  • Unexpired NY state driver's license or NYS ID
  • Knowledge of health insurance, Medicaid, Medicare and MLTCP
  • Experience working with a frail adult or elderly population with the ability to determine appropriate care plans and services
  • Time management, critical/creative thinking, communication, and problem-solving skills
  • Field experience assessing, planning, and evaluating member's/patient's care
  • Language preferences - Spanish, Russian, French, Creole, Mandarin, Cantonese
  • Demonstrated ability handling heavy caseloads
  • Intermediate Microsoft Word, Excel, and Outlook skills
Compensation

The hiring range for this position is $81,099 - $116,480 for Greater New York City Area (NY, NJ, CT residents) and $71,594 - $106,080 for All Other Locations (within approved locations). As a candidate for this position, your salary and related elements of compensation will be contingent upon your work experience, education, licenses and certifications, and any other factors Healthfirst deems pertinent to the hiring decision.
