Job Title: Home Health Nursing Assistant

We are seeking a skilled and compassionate Home Health Nursing Assistant to join our team at The University of Vermont Health Network. As a Home Health Nursing Assistant, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Job Summary:

The Home Health Nursing Assistant will assist patients with daily living activities, provide basic nursing care, and support the overall well-being of our patients. This is an excellent opportunity for a dedicated and caring individual to make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Assist patients with daily living activities, such as bathing, dressing, and grooming
  • Provide basic nursing care, including wound care, medication administration, and vital sign monitoring
  • Support patients with mobility and transfer activities, such as ambulation and use of adaptive equipment
  • Assist with meal preparation and special diets
  • Document patient care and services provided
  • Travel to and from patient homes and the University of Vermont Health Network Home Health & Hospice office
Requirements:
  • Graduation from a State-approved LNA course instruction
  • Previous experience in nursing home or similar facility desired
  • Ability to speak, read, and write in English
  • Valid driver's license and reliable vehicle with insurance
  • Ability to work independently and as part of a team
Working Conditions:

The Home Health Nursing Assistant will work in a variety of settings, including patients' homes, community-based settings, and outdoors. Travel may be required, and the ability to work during non-business hours may be necessary.

Essential Physical Requirements:
  • Ability to walk, sit, stand, bend, lift, and move intermittently during working hours
  • Ability to push, pull, and lift up to 50 pounds
  • Ability to navigate stairs of varying heights and degrees
  • Ability to maintain balance
  • Ability to kneel, crouch, stoop, twist, reach below waist level, reach overhead, and reach torso level
  • Manual finger dexterity to perform duties and use equipment
  • Sufficient eyesight to drive a car, use office equipment, and perform duties
  • Auditory acuity to assist individuals in person and by telephone
Performance Requirements:
  • Ability to perform basic nursing assistant and restorative duties
  • Ability to assist with personal care activities, including bathing, dressing, and grooming
  • Ability to document patient care and services provided
  • Ability to travel to and from patient homes and the University of Vermont Health Network Home Health & Hospice office
