Job Summary: We are seeking a RN Care Manager for our Roanoke location to bring comfort, compassion, and quality care to patients and families when they need it most. As an RN Care Manager, your ability to manage patient care and provide services requires substantial, specialized bedside nursing skills for symptom management, crisis intervention, and family education.
Responsibilities:
- Manage patient care and provide services with substantial, specialized bedside nursing skills for symptom management, crisis intervention, and family education.
- Develop and maintain rapport with patients and families, and work well with an interdisciplinary team.
- Travel to long-term care facilities, hospitals, and area agencies and businesses will be necessary.
Requirements:
- Graduate from an accredited school of nursing.
- Currently licensed to practice nursing by the Virginia State Board of Nursing.
- Two years' experience in acute care, home health, hospice, med/surg, or geriatric nursing.
- Current Virginia driver's license and driving record that meets the standards set forth in current policies.
Certification Requirements:
- BLS.
State License Requirements:
- Virginia.
On Call Requirements: Yes
