Responsibilities- Provide skilled nursing care to patients in their homes, focusing on promoting health, preventing illness, and alleviating suffering.
- Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to address these needs.
- Administer medications, perform treatments, and provide education to patients and families on disease management and self-care.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive and coordinated care.
- Document patient progress, including assessments, interventions, and outcomes, in accordance with regulatory requirements and company policies.
- Current licensure as a Registered Nurse (RN) in the state of employment.
- Minimum of 1 year of experience as a Home Health Nurse or in a related field.
- Ability to work independently and as part of a team, with strong communication and interpersonal skills.
- Strong assessment and problem-solving skills, with the ability to prioritize tasks and manage time effectively.
- Proficiency in electronic medical records (EMRs) and other healthcare software.
