Job Summary: We are seeking a skilled Licensed Practical Nurse (LPN) to join our home health care team. As an LPN, you will provide direct patient care services, evaluate patient needs, and assist in the preparation and execution of care plans. Responsibilities:

  • Provide direct patient care services, including administering medications, wound care, and vital sign monitoring.
  • Evaluate patient needs and develop individualized care plans to meet their unique requirements.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Document patient progress and report any changes or concerns to the Registered Nurse.

Requirements:

  • LPN license to practice in the state of employment.
  • 1+ years of patient care experience.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
