The estimated salary range for this role is $ $62,000 - $82,000 per year, based on industry standards and the location of Waukesha Memorial Hospital in Wisconsin.

This key position plays a vital role in coordinating patient services through an interdisciplinary process, providing a clinical and psychosocial approach throughout the continuum of care. Our ideal candidate will have experience with/knowledge of community resources, policies, and procedures, as well as 1+ years of clinical experience with/knowledge of age groups from unborn child to geriatric age groups.

About the Role:

  • Takes lead role in directing disposition of patients and utilization considerations
  • Assumes leadership role to facilitate interdisciplinary collaboration
  • Effectively problem-solves and actively pursues resolution
  • Directly communicates with staff, physicians, patients, and families
  • Role models leadership behavior through courtesy, respect, and efficiency

Requirements:

  • Associate's Degree (or higher) in Nursing
  • Current, unrestricted RN license in the state of employment
  • 1+ years of experience with/knowledge of community resources, policies, and procedures
  • 1+ years of clinical experience with/knowledge of age groups from unborn child to geriatric age groups

Preferred Qualifications:

  • Bachelor of Science
  • 2+ years of case management experience
  • Case Management Certification
  • Must have strong analytical, critical thinking, and organizational skills
