Role Overview for Physical Therapist (PT):

The Physical Therapist (PT) plays an essential role within the rehabilitation department of a skilled nursing facility, dedicated to improving residents' physical health and functional recovery.

Key Responsibilities:
  • Assessment and Evaluation: Conduct comprehensive physical assessments to identify residents' impairments and limitations, focusing on mobility, balance, and gait.
  • Individualized Care Plans: Develop tailored treatment plans based on assessments, with specific goals aimed at enhancing residents' mobility and reducing discomfort.
  • Section GG Data Collection: Accurately collect Section GG data critical for various assessments, ensuring data integrity for different evaluation types.
  • Treatment: Apply a variety of physical therapy techniques, including exercises and manual interventions, to enhance residents' physical abilities.
  • Education: Educate residents, family members, and caregivers on safe mobility practices and exercises for ongoing improvement. Utilize remote therapeutic monitoring for follow-up care post-discharge.
  • Documentation: Proficiently handle Point of Service (POS) care procedures and documentation. Maintain detailed records of evaluations, treatment plans, and progress notes in electronic medical records (EMRs).
  • Interdisciplinary Collaboration: Work closely with the rehabilitation team, including Occupational Therapists and Speech-Language Pathologists, to ensure comprehensive resident care.
