Job Summary:

Helio Health is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a vital role in the management and coordination of services to patients/residents participating in, or who have completed our treatment programs.

Key Responsibilities:

  • Maintain a case load and perform primary case work responsibilities including: screening, intake, orientation, assessment and intervention, treatment planning, referrals, crisis intervention, patient/resident education, case management, reporting and record keeping, consultation with other professionals, coordination of care with other community providers, discharge planning, and training of staff, volunteers, and students/interns.
  • Provide services according to agency policies, procedures, program guidelines, and governmental regulations.
  • Monitor provision of services when assigned.
  • Evaluate programs and identify strengths/weaknesses.
  • Assist in problem resolution and implementation of supervisor approved program modifications.

Requirements:

  • High School Diploma/GED required, associate's degree preferred.
  • Two years of experience working with behavioral health and/or substance abuse preferred.
  • Candidates with lived experience encouraged to apply.
  • Valid NYS Driver's License required.

Salary and Hourly Compensation:

Salary and hourly compensation ranges are provided in accordance with NYS law and are based on Helio Health's good faith belief of what is accurate at the time of posting.

Pay:

$24.00 per hour.
