Life Care Center of Citrus County is seeking a skilled Physical Therapist to join our team.

As a Physical Therapist with Life Care, you will have the opportunity to work in a dynamic and supportive environment, providing direct rehabilitation care to patients in accordance with applicable laws and regulations.

Job Summary: We are seeking a highly motivated and compassionate Physical Therapist to provide physical therapy services to patients in our post-acute care facility. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for delivering high-quality patient care.

About the Role: In this role, you will be responsible for assessing patients' needs, developing individualized treatment plans, and implementing evidence-based interventions to promote optimal outcomes. You will also collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions throughout the continuum of care.

Responsibilities:
  • Assess patients' physical abilities and develop personalized treatment plans to address their unique needs.
  • Implement evidence-based physical therapy interventions to promote functional mobility, strength, and endurance.
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure effective communication.
  • Conduct thorough evaluations and progress assessments to inform treatment decisions and adjust plans as needed.
  • Maintain accurate and detailed records of patient interactions, progress, and outcomes.


Requirements: To be considered for this role, you must possess a Bachelor's or Master's degree in Physical Therapy from an accredited program, hold current licensure in your state, and have at least one year of experience in post-acute care or a related setting. You should also be proficient in Microsoft Office and have excellent communication and interpersonal skills.
