BAYADA Home Health Care is seeking a skilled and compassionate Registered Nurse (RN) to join our team in providing high-quality home health care services to our clients in the Chestnut Hill/Manayunk/Roxborough area.
Job Responsibilities:- Develop and implement personalized care plans for clients, ensuring their unique needs are met.
- Provide hands-on care, including wound care, medication administration, and treatments, as prescribed by physicians.
- Conduct thorough assessments of clients' conditions, identifying areas for improvement and developing strategies for optimal care.
- Collaborate with clients, families, and healthcare teams to ensure seamless care transitions and effective communication.
- Document all client interactions, treatments, and outcomes in a timely and accurate manner.
- Current licensure as a Registered Nurse in Pennsylvania.
- Minimum one year of recent, verifiable clinical experience.
- Prior home care experience preferred, but not required.
- Ability to work independently, manage time effectively, and prioritize tasks.
- Strong interpersonal and communication skills, with a focus on empathy and compassion.
