Family Practice Nurse

We are seeking a highly skilled and compassionate Family Practice Registered Nurse to join our team at Progressive Community Health Centers, Inc.
Job Summary:The Family Practice Registered Nurse will play a vital role in providing high-quality patient care and support to our patients. This is an excellent opportunity for a dedicated and passionate nurse to make a meaningful difference in the lives of our patients.
Responsibilities:- Assess patient symptoms and provide guidance on the appropriate level of care
- Direct patients to emergency services when necessary and recommend follow-up care
- Review and triage lab and x-ray results, notifying healthcare providers of critical values
- Make follow-up calls to high-risk patients to assess changes in status and ensure follow-up treatment
- Process medication refills, changes, and prepare samples according to medical provider orders
- Perform patient education and instruction on medication regimens and documentation
- Administer medications and perform nurse visits, documenting all encounters accurately
- Act as a resource to medical support staff and collaborate with medical staff in carrying out delegated patient care activities
- Associate Degree in Nursing or Bachelor of Science in Nursing (preferred)
- Currently licensed in the State of Wisconsin
- Two years of general nursing experience in a clinic or hospital setting, with experience with patients of all age groups preferred
- Experience in a Community Health Center, a plus
- Experience with EPIC is preferred
