- Monitor patient progress, adjust care plans as needed, and provide ongoing support to ensure optimal outcomes.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
- Diploma, Associate, or Bachelor's degree in Nursing
- Minimum of one year of nursing experience, preferably in home health or a related field
- Strong clinical skills, with a focus on gerontology and chronic disease management
- Current and unrestricted RN licensure
- Current CPR certification
- Valid driver's license and reliable transportation
