We are looking to expand our PRN team in multiple regions. Ideal candidates should possess prior experience in conducting virtual preventative health assessments and wellness coaching for adult clients.
KEY RESPONSIBILITIES
- Perform individual virtual health assessments utilizing telehealth technology.
- Inform participants about their health risk factors related to chronic conditions and guide them towards suitable programs based on their employer's offerings.
- Utilize electronic medical records (EMR) to document and manage all consultations.
- Evaluate health risks, provide clinical advice, and develop personalized action plans tailored to each participant's outcomes.
- Assess mental health status and refer patients to appropriate resources based on their needs.
- Connect participants with employee assistance programs based on their specific health profiles.
- Encourage participants to actively participate in their health improvement plans.
- Master of Science in Nursing is mandatory; Doctor of Nursing Practice is preferred.
- Valid RN licenses in relevant states.
- Current CPR certification.
- A minimum of one year of experience as a Nurse Practitioner in a Primary Care or Internal Medicine environment.
- Proficient computer skills, including the ability to engage with patients via video while documenting.
- Strong communication and interpersonal abilities to effectively consult and engage patients.
- Bilingual proficiency in English and Spanish is advantageous.
