About the Role

We are seeking a highly motivated and compassionate Registered Nurse (RN) to join our Nurse Residency Program in Home Health. As a RN Graduate, you will have the opportunity to develop your skills and knowledge in caring for patients in the home setting.

Responsibilities
  • Collaborate with the clinical manager and peer mentor to provide home health nursing services to assigned patients.
  • Develop and implement care plans in collaboration with patient care teams.
  • Educate families on patient condition, needs, and care.
  • Perform head-to-toe physical assessments, check vital signs, and manage medications.
  • Administer injections, IV therapy, or tube feedings, and maintain catheters.
  • Complete documentation timely and accurately.
  • Supervise LPNs and HHAs, once deemed competent.
  • Regularly communicate patient progress to the clinical manager, physician, and care team.
Qualifications
  • Current RN license, specific to the state(s) assigned to work.
  • Current CPR certification.
  • Less than one year of clinical RN experience.
  • Valid driver's license and reliable transportation, with required liability insurance.
