Position Overview

The Clinical Pharmacist at Ocean Health Initiatives will be responsible for delivering safe and effective pharmaceutical services, including the dispensing of medications and providing comprehensive patient care.

Key Responsibilities

1. **Patient Education**: Educate patients on their medications, including usage, dosage, side effects, and expected therapeutic outcomes. Utilize educational materials tailored to the patient's health literacy level.

2. **Medication Dispensing**: Accurately fill prescriptions in compliance with federal and state regulations, ensuring all in-house and external prescriptions are processed correctly.

3. **Clinical Collaboration**: Act as a resource for healthcare providers regarding medication management, pharmacokinetics, and drug interactions. Assist in managing patients with multiple medications through medication reconciliation and comprehensive medication lists.

4. **Patient Monitoring**: Work closely with the Coumadin Management Team to monitor patient INR levels and ensure timely medication renewals.

5. **Quality Assurance**: Conduct thorough chart reviews and collaborate with healthcare providers to discuss findings and improve patient outcomes.

6. **Inventory Management**: Maintain accurate inventory levels of medications and supplies, ensuring timely procurement and minimizing stock shortages.

7. **Documentation**: Ensure meticulous documentation of all clinical activities and communications with patients and healthcare professionals.

8. **Professional Development**: Stay updated on the latest pharmacotherapy practices through continuous education and training.

Qualifications

- Graduate of an ACPE accredited pharmacy program (BSPharm or PharmD).
- Licensed Pharmacist in New Jersey.
- Experience in ambulatory care and completion of a PGY-1 residency.
- Certified in Basic Life Support (BLS).
