Job Summary:
We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team of healthcare professionals. As an RN with VNA & Hospice of the Southwest Region Inc., you will play a vital role in assessing patient needs, developing and implementing individualized care plans, and providing exceptional patient care.
Key Responsibilities:
- Assess patient needs and develop comprehensive care plans in collaboration with the healthcare team.
- Provide high-quality, patient-centered care, including administering medications, wound care, and other medical interventions as needed.
- Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination and communication.
- Document patient progress, including medical history, treatment plans, and outcomes.
- Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
- Current RN licensure in the state of Vermont.
- 2+ years of experience in home health and hospice care, preferably in a similar role.
