About the Role: We are seeking a skilled Physical Therapist to join our team in Two Rivers, Wisconsin. As an Acute Care Specialist, you will be responsible for providing high-quality physical therapy services to patients in need of ongoing care.

Job Summary: The ideal candidate will have a Bachelor's Degree in Physical Therapy and be licensed to practice in the state of Wisconsin. You will interpret physician referrals, complete evaluations, and develop individualized treatment plans to help patients achieve their goals. Key Responsibilities:

  • Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
  • Establish a plan of care and implement individual treatment based on evaluation results and functional goals.
  • Monitor and evaluate outcomes and adjust the individualized treatment plan as needed.
  • Plan for and complete patient discharge, including providing written home programs and referrals for community services.

Requirements: To be successful in this role, you will need to have intermediate computer skills, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment. You must also be able to lift up to 50 lbs and push/pull with 30 lbs of force.

Salary Information: The estimated salary for this role is $90,000 - $110,000 per year, depending on experience and qualifications. This figure includes guaranteed hours, holiday pay, continuing education opportunities, and a comprehensive benefits package.
