Job Title: Case Manager III

Job Summary:

We are seeking a highly skilled and compassionate Case Manager III to join our team at Bayview Hunters Point Foundation. As a key member of our social services team, you will be responsible for providing comprehensive case management services to individuals and families experiencing homelessness. Your expertise and dedication will play a critical role in helping our clients achieve stability and self-sufficiency.

Key Responsibilities:
  • Manage a caseload of complex cases, providing direct support and intervention to clients.
  • Conduct thorough assessments of clients' needs, strengths, and challenges, and develop individualized care plans in collaboration with clients and other service providers.
  • Facilitate access to necessary services, including healthcare, housing, education, employment, and social support.
  • Supervise and mentor a team of case managers, providing guidance and professional development opportunities.
  • Ensure all case management activities comply with organizational policies, legal requirements, and ethical standards.
Requirements:
  • Masters degree in a related field or 6 years of experience in direct case management.
  • Excellent interpersonal and communication skills to establish rapport, actively listen, and effectively communicate with clients, colleagues, and external stakeholders.
  • Basic knowledge and understanding of homelessness issues, social services, and community resources in San Francisco.
  • Certification in case management (e.g., CCM, ACM) is preferred.
