Job Summary

Job Responsibilities
  • Deliver high-quality patient care in the comfort of patients' homes, utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation.
  • Provide comprehensive care to patients with complex medical needs, including post-surgical recovery and illness management.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and patient outcomes.
  • Identify and link patients with community resources to facilitate referrals and respond to social service needs.
  • Track and support patients when they obtain services outside the practice, such as Emergency Departments, Hospitals, and other healthcare facilities.
  • Follow-up with patients within a few days of an emergency room visit or hospital discharge.
  • Communicate test results and care plans to patients/families under the direction of the practitioners.
  • Provide relevant self-management support for patients with chronic illnesses as identified by clinical teams.
Requirements
  • Graduate of an accredited nursing program
  • Current nursing license, in good standing with the State licensing board
  • Minimum of one (1) year HH experience
  • Oasis Start of Care Preferred (Will train RN with HH exp)
