Job Summary

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to adult and geriatric clients in their homes. As a member of our multidisciplinary healthcare team, you will be responsible for assessing clients' needs, developing and implementing individualized care plans, and providing manual therapeutic exercises, gait training, and other interventions.

Responsibilities
  • Conduct home visits to assess clients' needs and develop personalized care plans.
  • Implement evidence-based interventions, including manual therapeutic exercises, gait training, and balance assessments.
  • Educate clients and their families on rehabilitative care and activities to promote independent living.
  • Accurately document client care management and services provided.
Requirements
  • A current Maryland Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
