Job Summary

We are seeking a highly skilled and compassionate Physical Therapist to join our team at Pinnacle Home Care Inc. as a Full-time Physiatrist. As a key member of our interdisciplinary team, you will be responsible for providing high-quality physical therapy services to patients in their own homes.

Key Responsibilities
  • Assess patients' conditions and develop personalized treatment plans to improve mobility, reduce pain, and restore function.
  • Deliver therapy to patients in their homes, adapting treatment plans to suit home environments while ensuring high-quality care and patient safety.
  • Collaborate with the interdisciplinary team to develop and implement client care plans, ensuring seamless care coordination.
  • Teach, supervise, and counsel patients and their families on physical therapy programs, including the use of adaptive devices.
  • Record evaluation data, treatments, and client responses to therapy intervention, as well as quality assurance data as directed.
  • Participate in case conferences, staff meetings, and in-service programs to stay up-to-date on best practices and industry developments.
Requirements
  • Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
  • Licensed or registered by the State.
  • At least one year of supervisory experience in home care physical therapy.
