Overview

Responsibilities
  • Conduct comprehensive health risk assessments to identify active and chronic disease conditions, as well as physical, mental, and social needs.
  • Review and address preventative needs for patients, educating and counseling them on identified conditions.
  • Make recommendations and communicate findings to Primary Care Physicians through documentation of outcomes.
  • Collaborate with interdisciplinary teams, including physicians, pharmacists, nurse case managers, and behavioral health specialists, to ensure high-quality patient care.
  • Order, conduct, and interpret diagnostic, lab, and radiology services as needed.
  • Teach and counsel individuals, families, and groups on health-related topics.
  • Serve as a healthcare resource, interdisciplinary consultant, and patient advocate.
Credentials Required

Nurse Practitioner: Masters degree in nursing, New Mexico license, and ANCC, AANP, AGACNP, or AGNP certification. At least one year of experience in caring for Medicare Advantage members is preferred.

Physician Assistant: Baccalaureate prepared CAHEA Certified Physician Assistant or two years of working experience as a Certified Physician Assistant, and licensed by the New Mexico Board of Medical Examiners.
