Pinnacle Home Care Inc. is seeking a skilled Speech Therapist to join our team on a PRN basis. As a Speech Therapist PRN, you will provide high-quality Speech Therapy services to patients in their homes, working closely with our interdisciplinary team to deliver exceptional patient care.

Responsibilities
  • Evaluate patients' level of functioning and hearing, recommending mechanisms to enhance their hearing ability.
  • Conduct comprehensive assessments of patients' needs, including OASIS assessments, to develop effective treatment plans.
  • Establish and revise Speech Therapy treatment plans in consultation with physicians, ensuring patient-centered care.
  • Collaborate with the interdisciplinary team to develop and implement client care plans, addressing Speech Therapy needs.
  • Communicate effectively with patients, families, and healthcare team members, providing education and support as needed.
  • Record and report patient progress, reactions to treatment, and any changes in condition to physicians.
  • Participate in periodic revisions of physician plans of treatment, ensuring patient needs are met.
Qualifications
  • Graduate of an accredited Speech Therapy program or equivalent education.
  • Licensed in the State of Florida as a Speech Therapist.
  • Two years of clinical experience as a Speech Therapist.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
