Job Title: RN/LPN PRN

We are seeking a skilled and compassionate Registered Nurse/Licensed Professional Nurse (RN/LPN) to join our team at Heartland Community Health Clinic. As a key member of our healthcare team, you will provide high-quality patient care, coordinate day-to-day activities, and ensure seamless communication with patients, families, and physicians.

Key Responsibilities:
  • Manage acute, chronic, and episodic high-risk patients of all ages, with a focus on improving or maintaining their current health state.
  • Oversee patient evaluation, treatment decisions, and consultations, ensuring adherence to Heartland Health Services' mission, strategic goals, and regulatory requirements.
  • Develop and implement standards, policies, procedures, protocols, and guidelines to enhance patient outcomes and satisfaction.
  • Provide education to patients and family caregivers on healthcare resource utilization and document patient interactions.
  • Coordinate services with other patient care locations, maintaining effective communication with allied services.
  • Assist providers with technical procedures, prepare patients, and administer medications as directed.
  • Document patient records, charts, and reports, ensuring accurate and timely information.
  • Ensure informed consent and communicate pertinent patient information to the healthcare team.
  • Review patient charts to ensure quality assurance and maintain equipment and supplies in a safe and efficient manner.
Requirements:
  • Current State license as a Registered Nurse or Licensed Professional Nurse.
  • Ability to lift up to 50 pounds.
  • Demonstrate knowledge and skills necessary to provide patient care appropriate to the age of the patients served.
  • Previous experience in an office setting or one-year experience in an acute care setting preferred.
  • Strong interpersonal skills necessary for interacting with patients, families, and physicians.
