As a Registered Nurse, you'll play a vital role in our team, providing personalized care and support to patients with chronic conditions or recovering from illness, injury, or surgery.

Key Responsibilities:
  • Provide skilled nursing visits to patients in their homes, assessing their needs and developing individualized care plans.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Administer medications, perform treatments, and provide education to patients and caregivers on disease management and self-care.
  • Conduct thorough assessments, documenting patient progress and making recommendations for further care as needed.
  • Work closely with our interdisciplinary team to ensure comprehensive care and support for patients and their families.

Requirements:
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum one year of nursing experience, preferably in home health or a related field.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.
