Job Summary

We are seeking a skilled RN Case Manager to join our team at the Transgender Health Medical Evaluation Unit. As a RN Case Manager, you will be responsible for providing nursing expertise and case management services to patients undergoing gender-transition.

Key Responsibilities
  • Participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care.
  • Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring.
  • Develop and implement local strategies using inpatient, outpatient, onsite and telephonic CM.
  • Develop and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, VIRTUAL HEALTH MODALITIES, and databases for community resources.
  • Maintain liaison with appropriate community agencies and organizations.
  • Accurately collect and document patient care data.
  • Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.
  • Establish mechanisms to ensure proper implementation of patient treatment plan and follow-up post discharge in ambulatory and community health care settings.
  • Provide appropriate health care instruction to patient and/or caregivers based on identified learning needs.
  • Provide transgender education related to clinical and/or case management nursing expertise to healthcare professionals as needed to include, but not limited to, didactic lectures, case-based learning, virtual grand rounds, and precepting students/residents/ fellows, across small and large group settings to include conferences.
  • Provide nursing services within the scope of basic RN knowledge/skills/abilities to support unit function as needed.
  • Travel to DOD/DHA locations to provide education and training to clinical personnel within the scope of HCWs specific expertise in caring for the THMEU population.
Requirements
  • Bachelors of Science in Nursing and Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE).
  • Certifications in addition to Basic Life Support : Possess one of the following certifications: Commission for Case Manager Cer ification Certified Case Manager (CCM), Certified Disability Management Specialist (CDMS), Certified Rehabilitation Registered Nurse (CRRN), Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S), Advanced Certification in Continuity of Care (ACCC), Certified Rehabilitation Counselor (CRC), Nurse Case Manager (RN-NCM), Care Manager Certified (CMC).
  • A minimum of 2 years as a RN with full-time experience in utilization management/utilization review or in broad-based clinical nursing and clinical case management working with adults, children, families, seniors and groups within the last 3 years.
  • Current, full, active, and unrestricted license as a Registered Nurse.
  • Must attend continuing education annually to ensure continued awareness and expertise in clinical advances and best practices for gender-affirming healthcare.
