**About the Role:**

We are seeking an experienced Hospice Registered Nurse Case Manager to join our team in the Topeka, KS area. This role is a fantastic opportunity for a dedicated healthcare professional to make a meaningful difference in the lives of patients and their families.

Job Summary:

The RN Case Manager will provide compassionate nursing care to clients with terminal illnesses, working closely with the interdisciplinary team to develop and implement personalized plans of care that prioritize quality of life.

Responsibilities:

  • Conduct comprehensive assessments and evaluations to determine patient needs and goals of care.
  • Collaborate with attending physicians and the Hospice Medical Director to ensure timely and effective treatment planning.
  • Coordinate hospice care services, ensuring seamless communication between patients, families, and healthcare professionals.
  • Provide education and support to patients and their loved ones regarding the dying process, plan of care, and end-of-life decision-making.

Requirements:

  • Kansas or Compact Nursing License.
  • At least one (1) year of Hospice experience.
  • Valid driver's license and auto liability insurance.

Compensation:

This full-time position offers a competitive salary range of $ $85,000 - $110,000 per annum, depending on experience, as well as a benefits package, paid mileage, and opportunities for professional growth and development.
