We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in providing high-quality patient care and collaborating with our interdisciplinary team to ensure optimal patient outcomes.
Responsibilities:
- Develop and implement individualized physical therapy plans to meet the unique needs of each patient.
- Conduct thorough assessments and evaluations to identify patient goals and objectives.
- Provide direct patient care, including exercises, modalities, and education.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
- Graduate of an accredited physical therapy program.
- Current licensure to practice as a physical therapist in the state of operation.
- 2+ years of experience in adult physical therapy, with a focus on home health.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
