Job Summary

We are seeking a highly skilled Pharmacist Staff to join our team at St. Elizabeth Healthcare. As a key member of our pharmacy team, you will be responsible for providing pharmaceutical care to patients, with a focus on clinical monitoring and therapeutic interventions that improve patient outcomes.

Key Responsibilities
  • Medication Order Verification: Verify medication orders from licensed prescribers, ensuring they meet the requirements of a complete medication order. Assess drug interactions, allergy alerts, duplication of therapy, and potential therapeutic interactions.
  • Therapeutic Clinical Monitoring: Actively participate in the ongoing monitoring of medication therapy, in compliance with approved Collaborative Care Agreements and medication management protocols. Direct medication therapy within the bounds defined by Collaborative Care agreements and protocols.
  • Compounds and Dispenses Pharmaceuticals: Ensure timely dispensing of appropriate quantities of the correct medication. Supervise compounding according to USP standards, verification of ingredient compatibility, verification of content, and verification of appropriate labeling.
  • Supervises and Directs Interns and Technicians: Provide active direct supervision and mentoring of Interns and Technicians, as guided by ICARE values. Monitor timely completion of tasks, redirect staff to meet patient needs, and encourage use of the AIDET model in interaction with staff, patients, and visitors.
  • Medication Use Safety Initiatives: Detect and report variances in dispensing on internal variance report forms or in Midas as is appropriate. Identify opportunities and participate in departmental efforts to increase the safe use of medications.
  • Controlled Substance Accountability: Participate in and supervise the receipt, dispensing, and distribution of controlled substances through the use of Pyxis C-II Safe, Pyxis Medstation 4000, internal perpetual inventories, Pyxis Anesthesia Stations, Anesthesia Boxes, and anesthesia waste logs.
  • Contributes to the Positive Overall Financial Status of the Department: Encourage prescriber compliance with formulary restrictions and therapeutic interchanges. Assure formulary adherence and process non-formulary requests in the appropriate manner.
  • Drug Information / Staff Development: Provide accurate, adequate, and timely drug information to the medical staff, nursing staff, and patients. Serve as a preceptor or provide advice to Pharmacy Residents and Pharmacy Interns.
  • Maintains Professional Competency: Maintain licensure, complete all departmental competency/skills assessment requirements, and maintain current knowledge of pharmacy practice.
Qualifications
  • Education, Credentials, Licenses: Graduate from ACPE accredited pharmacy college, current US pharmacist license in the state where work is being performed.
  • Specialized Knowledge: Problem-solving and critical thinking skills, good communication skills, familiar with Current Practice Standards (ASHP).
  • Kind and Length of Experience: Previous hospital experience.
  • FLSA Status: Non-Exempt.
