We are seeking a skilled RN Complex Case Manager to join our team at UF Health Shands. As a key member of our Transitions of Care model, you will play a critical role in assessing, planning, implementing, and monitoring comprehensive care plans for patients.
Key Responsibilities- Collaborate with healthcare teams to identify discharge barriers and complex patients requiring focused care management
- Develop and implement care plans to prevent 30-day readmissions and improve patient outcomes
- Provide education on disease management and medication adherence
- Work with post-acute care teams to ensure seamless transitions of care
- Communicate effectively with patients, families, and healthcare providers to ensure optimal care
- Licensed RN in Florida with 2 years of recent RN experience
- BSN preferred; MSN or BSN within 3 years of employment required for Case Managers hired after January 1, 2018
- Professional certification in specialty area preferred
- Valid RN license in the State of Florida
