Position Overview

The Rehabilitation Licensed Practical Nurse (LPN) is responsible for delivering direct nursing care to assigned patients under the guidance of a registered nurse (RN) within patient care departments.

Key Responsibilities
  • Medication Administration: Administers prescribed medications and manages IV piggyback antibiotics and fluids, while excluding high-risk medications.
  • Care Coordination: Engages with healthcare providers to obtain orders and facilitate comprehensive patient care.
  • Patient Assessment: Conducts thorough assessments and documents findings for assigned patients.
  • Dressing Changes: Performs dressing changes as per medical directives.
  • Trach Care: Provides tracheostomy care as required.
  • Tube Management: Oversees NG, g-tube, and j-tube care, including administering feedings and medications as prescribed.
  • Ostomy Care: Responsible for the care and management of ostomy patients.
  • Patient Education: Reinforces educational materials and discharge instructions to patients and their families.
  • Care Planning: Collaborates with multidisciplinary teams, including providers and therapists, to create tailored care plans.
  • Vital Signs Monitoring: Monitors vital signs and provides essential hands-on nursing care.
