- Care Coordination: Engages with healthcare providers to obtain orders and facilitate comprehensive patient care.
- Patient Assessment: Conducts thorough assessments and documents findings for assigned patients.
- Dressing Changes: Performs dressing changes as per medical directives.
- Trach Care: Provides tracheostomy care as required.
- Tube Management: Oversees NG, g-tube, and j-tube care, including administering feedings and medications as prescribed.
- Ostomy Care: Responsible for the care and management of ostomy patients.
- Patient Education: Reinforces educational materials and discharge instructions to patients and their families.
- Care Planning: Collaborates with multidisciplinary teams, including providers and therapists, to create tailored care plans.
- Vital Signs Monitoring: Monitors vital signs and provides essential hands-on nursing care.
