Healthcare Counselor

We are seeking a highly skilled Healthcare Counselor to join our team at UNC Health Pardee BlueMD. This role will involve working in a dynamic team-based environment, providing counseling services and ongoing care management support to patients in an outpatient medical office setting.

As a Patient Advocate, you will be responsible for delivering high-quality care, developing relationships with community health and social agencies, and referring patients to appropriate resources.

• Provide counseling services to patients and their families, addressing social, psychological, and family situations.
• Collaborate with medical providers, care management, and the clinical team to enhance patient management and satisfaction.
• Develop and maintain relationships with community health and social agencies, monitoring resources for meeting patient needs.
• Communicate effectively with interdisciplinary care teams, documenting necessary information regarding patients and families.

To succeed in this role, you will need:

• Licensed Clinical Social Worker (LCSW) certification.
• A Master's degree in Social Work (MSW) from a program accredited by the Council on Social Work Education.
• BLS certification.

This full-time position offers a competitive salary, ranging from $65,000 to $85,000 per year, depending on experience.
