We are seeking a highly skilled and motivated Pharmacist to join our team at PharMerica. As a key member of our pharmacy team, you will play a critical role in providing exceptional patient care and support to our patients in long-term care settings.
Responsibilities- Evaluate physician medication orders for appropriateness, making recommendations for therapy changes as needed.
- Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medications.
- Ensure timely and accurate ordering, evaluation, and discussion of laboratory tests for high-risk conditions.
- Collaborate with the healthcare team to provide clinical and technical support as needed.
- Document all communication with physicians, nurses, and other healthcare team members in the medical record.
- Implement policies and procedures for safe medication procurement, storage, distribution, and use, as well as disposal of medications.
- Inform the Pharmacy Manager of medication-related errors and assist facility/agency staff with procedures related to safe and effective medication management.
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- One year experience in long-term care.
- Licenses/Certifications as required by government regulations and business needs.
- Detail-oriented, goal-oriented, team player, and excellent oral communication skills.
