The Clinical Pharmacist is a key member of our team, responsible for providing safe and effective pharmacy services to our patients in the comfort of their own homes. Responsibilities- Determine patient suitability for home care services, assessing medical conditions and prescribed medication therapy.
- Design, recommend, and evaluate patient-specific pharmacy care plans for home care patients.
- Collect and organize patient-specific information to prevent, detect, and resolve medication-related problems.
- Provide medication-use education to patients, caregivers, and healthcare professionals.
- Ensure continuity of pharmaceutical care across patient-care settings.
- Prepare and dispense medications, adhering to safety and quality assurance practices.
- Manage medication administration equipment and troubleshoot issues.
- Participate in incident reporting and quality assurance activities.
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license as granted by the State Board of Pharmacy.
- At least one year of experience in sterile IV admixture procedures, or equivalent experience as approved by the Vice President of Pharmacy and/or Sr. Vice President of Clinical Services.
- Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
- Experience in home care, hospital, or other clean room settings.
- Competitive salary range: $105,226.27-$175,391.83.
