We are seeking a highly skilled Home Health Assistant to join our team at Geisinger Health. This is an exciting opportunity to work in a clinical environment, providing personalized care and support to individuals in their homes.

About the Role:
  • As a Home Health Assistant, you will provide hands-on care and assistance to patients, including personal care, nutrition, and hydration, as well as assist with ambulation and transfers.

You will work independently, following a care plan developed by the home care coordinator and patient, to maintain a healthy and independent lifestyle in the community.

Responsibilities:
  • Provide direct patient care, including but not limited to: Personal care, nutrition, and hydration, obtain vital signs, assist with ambulation and transfers of patient, and active and passive range of motion.
  • Participate in indirect patient care responsibilities, including but not limited to: Light housekeeping, assist with laundry, use supplies accurately and economically, and delivery of medication and supplies as needed.
  • May require on-call coverage for after-hours care for emergencies in home.
  • Remain attentive to patient and personal care assistant safety responsibilities, including: Appropriately utilizing good body mechanics and use of gait belt, back brace for transfers and ambulation of patients.
  • Collect data for mandatory requirements and various quality improvement projects and actively involved in action plans and implementation to improve quality for the Geisinger program.
  • Complete accurate and thorough documentation for services provided according to program policy.
  • Maintain effective communication skills with the older adult and with co-workers.
  • Explain actions to patient before starting task.
  • Understand and demonstrate that all care is done with full regard to patient's rights.
  • Provide orientation and redirection to patient.
  • Promote independence in patient by offering choices and fostering self-help skills.
  • Communicate respect and caring verbally and non-verbally with patient and other staff.
  • Work cooperatively with other members of the interdisciplinary team to provide quality of care to Geisinger patients.
  • Attend assigned interdisciplinary team and scheduled monthly meetings and complete all required educational trainings.
Requirements:
  • High School Diploma or Equivalent (GED) - Required
  • Minimum of 1 year Related work experience - Preferred

Estimated Salary:

$45,000 - $55,000 per year
