Job SummaryWe are seeking a skilled Hospice Care Specialist to join our team. As a Hospice Care Specialist, you will play a vital role in providing comfort care to patients, enabling them to remain at home with their loved ones. You will work collaboratively with our interdisciplinary team to meet each patient's unique physical, emotional, and spiritual needs.
Key Responsibilities- Provide comfort care to patients, including assistance with daily living activities, medication management, and emotional support.
- Work as a member of the hospice team to develop and implement individualized care plans that address each patient's unique needs.
- Collaborate with healthcare professionals, family members, and caregivers to ensure seamless care coordination.
- Document patient progress, including medical history, treatment plans, and outcomes.
- Participate in ongoing education and training to stay current on best practices in hospice care.
- Certified Nursing Assistant (CNA) or equivalent experience preferred.
- Current CPR certification required.
- State-specific certifications or licenses may be required (e.g., California, Colorado, New Hampshire, New Jersey, Virginia, Wisconsin).
