About This Role

Bright Paths Home Health Agency is seeking a compassionate and dedicated Caregiver to provide personalized care services in patients homes.

Responsibilities:
  • Deliver quality care services in patient residences, adhering to established care plans
  • Perform household tasks, ensuring a clean and safe living environment
  • Transport patients to medical appointments or hospitals as needed
  • Administer prescribed medications under the supervision of healthcare professionals
  • Assist with daily living activities, promoting independence and dignity
  • Maintain accurate records of patient care, documenting provided services
  • Collaborate with healthcare teams to ensure continuity of care
  • Support patients with mobility needs, facilitating physical therapy and exercise
Requirements
  • Certified Caregiver or relevant experience in senior care
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Compassion, empathy, and a patient-centered approach
