About the Role

We are seeking a highly skilled and experienced MDS Coordinator RN to join our team at Terraces of Boise, a HumanGood community. As a key member of our care team, you will play a vital role in ensuring the highest quality of care for our residents.

Key Responsibilities
  • Develop and Implement Resident Care Plans: Collaborate with the interdisciplinary team to develop and implement comprehensive resident care plans that meet the unique needs of each individual.
  • Conduct Resident Assessments: Conduct thorough resident assessments to identify care needs and develop effective care plans.
  • Lead PPS Meetings: Lead PPS meetings to ensure that skilled residents receive the appropriate level of care and that billing aspects of Medicare are thoroughly documented and reimbursed.
  • Supervise LVN MDS Nurses: Provide direct supervision to LVN MDS nurses to ensure that they are providing high-quality care to residents.
  • Review CMS Reports: Regularly review CMS reports to analyze data and identify areas for quality improvement.
  • Collaborate with DON: Collaborate with the Director of Nursing (DON) to manage survey-related reports and ensure compliance with regulatory requirements.
  • Stay Current with RAI Updates: Participate in HumanGood-provided Medicare and MDS webinars and education to stay current with RAI updates and evidence-based practices in post-acute long-term care settings.
Requirements
  • Completion of Accredited Licensed Nursing Program (RN): Must have a current RN license and completion of an accredited nursing program.
  • Minimum Three Years' Experience as an RN: Must have a minimum of three years' experience as an RN, with a minimum of one year of experience as an MDS Coordinator RN.
